COVID-19 self-assessment

Last updated: July 28, 2021

Version 5.1
Updated guidance for people who are fully vaccinated.

Take this self-assessment if you were exposed to COVID-19 (novel coronavirus) or have symptoms.

You will get a recommendation on what to do next.

You can also take it on behalf of someone else.

If this is a medical emergency, call 911. Advise them of your symptoms and if you have recently travelled.

Everyone in your household who is not fully vaccinated must stay home if anyone has COVID-19 symptoms or is waiting for test results after experiencing symptoms.

Stay home until the person with symptoms gets a negative COVID-19 test result, or is cleared by public health, or is diagnosed with another illness.

The self-assessment is only meant as an aid and cannot diagnose you. Consult a health care provider if you have medical questions.

Visiting a nursing or long-term care home?

Some homes may need you to get a COVID-19 test before visiting. For long-term care homes, you do not need to get tested before an outdoor visit with a loved one.

Did you get a COVID-19 test?

Your result may be available online if you have a photo (green) health card.
